Purpose:
To evaluate visual and refractive outcomes of phakic intraocular lenses (PIOLs) in eyes affected by different degrees of keratoconus. To investigate the predictability and stability of this surgery. To compare results achieved with two different models.

Vissum Corporación. Alicante (Spain)
Methods:
52 eyes of 34 patients diagnosed with stable keratoconus were implanted with two main types of PIOLs: 20 iris-claw (Artiflex) and 28 posterior chamber implantable collamer lenses (ICL). A complete ophthalmologic examination was performed pre and postoperative. The main outcome measures of the study were postoperative changes from the preoperative manifest refraction, uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), corneal topography and endothelial cell count (ECC). Alpins analysis was also performed.
Results:
Spherical equivalent (SE) changed from -9.31±4.20 (-20.75 to -2.75) preoperative to -0.46±0.88 (-3.25 to 1.00) postoperative (P<0.001). None of the patients suffered loss of lines in CDVA in Snellen scale. UDVA changed from 0.08±0.09 to 0.71±0.26 (P<0.001), CDVA changed from 0.77±0.24 to 0.87±0.98 (P<0.001). Efficacy and safety indexes were 0.90±0.26 and 1.19±0.29, respectively. No significant changes in refraction were detected after the surgery during the follow up of any of the cases.
Conclusions:
PIOLs implantation is a suitable refractive surgical option for stable keratoconus cases. This surgery may be specially indicated for the management of high ametropias and shows good levels of efficacy and safety. Refractive and keratometric stability was confirmed during the follow up of the investigation. The comparison between ICL and Artiflex revealed slightly higher values of efficacy for the last one.
