Abstract Details

Purpose:

To determine changes in visual acuity, contrast sensitivity and color vision, as well as their effect on the subjective visual function after Descemet membrane endothelial keratoplasty (DMEK) in patients suffering from bilateral Fuchs endothelial dystrophy (FED).

Setting:

The Netherlands Institute for Innovative Ocular Surgery (NIIOS). Rotterdam, The Netherlands.

Methods:

A total of 29 patients with a history of bilateral FED and unilateral DMEK were included in our study, as well as 11 control subjects. Study participants were divided in three groups (phakic (n=12), pseudophakic (n=17), control (n=11)). Patients with unilateral cataract, unilateral IOL, previous anterior segment surgery or any other concurrent ocular disorder that may limit best corrected visual acuity (BCVA) under 20/40 (0.5) were excluded. Pelli Robson contrast sensitivity test and Fansworth Munsell 100 Hue color vision test were used to assess contrast sensitivity and color vision respectively. A specific questionnaire consisting of 11 questions was developed to measure the subjective visual performance. Individual variability was taken into account by measuring all visual parameters for the untreated (FED) and the treated eye (DMEK) of each subject.

Results:

Comparison between both eyes of each patient showed significant difference in contrast sensitivity between FED and DMEK eyes (P=0.000). Contrast sensitivity in pseudophakic DMEK eyes was lower than in the control group (P=0.045), there was no difference between phakic DMEK eyes and control group (P=0.133). No difference was found in color vision between FED and phakic or pseudophakic DMEK eyes (P=0.802 and P=0.227 respectively). Subjective visual satisfaction was better for DMEK than for FED eyes (P=0.013 and P=0.000, phakic and pseudophakic respectively).

Conclusions:

For patients with bilateral FED, contrast sensitivity improved after DMEK compared to the untreated fellow eye. Improvement of those parameters after surgery influenced the subjective perception of patientsÂ’ visual quality.
