Abstract Details

Purpose:

A combination of collagen cross-linking (CXL) and surface ablation, simultaneously or serially, may be a promising means to correct part of the irregular astigmatism in corneal ectasia. Accordingly, surgeons will be confronted with patients that will present with previously cross-linked corneas requiring excimer laser ablation. Currently, the ablation rate per pulse in a cross-linked cornea is unknown.

Methods:

The laser excimer ablation rate of porcine corneas was analyzed using optical coherence pachymetry (OCP). The following conditions were analyzed: 1) normal cornea, with 5 consecutive ablations of 50 µm each. 2) normal cornea, previously soaked with riboflavin 0.1 %, cross-linked (18mW/cm2), and subjected to 5 consecutive ablations of 50 µm each. 3) normal cornea, previously soaked with riboflavin 0.1 %, cross-linked (3mW/cm2), with 5 consecutive ablations of 50 µm each.

Results:

The following ablation rates were obtained For no CXL: - 1X 50 µm 60 - 2X 50 µm 56 For CXL at 18 mW/cm2: - 1X 50 µm 53 - 2X 50 µm 44 For CXL at 3 mW/cm2: - 1X 50 µm 48 - 2X 50 µm 39

Conclusions:

CXL seems to decrease the ablation rate of the cornea, this decrease seems to be more pronounced in a low fluence CXL (3 mW/cm2)
