The Haigis L solution for challenging post LASIK IOL patients

Abstract Details

Purpose:

To analyze the clinical outcomes in post LASIK patients undergoing uncomplicated cataract surgery.

Setting:

All patients underwent IOL calculation evaluation using the IOLMaster500 in an outpatient office setting, and all patients underwent uncomplicated cataract surgery in an approved outpatient ambulatory surgery center.

Methods:

A retrospective review of 40 eyes with known prior myopic or hyperopic LASIK of at least one year postop underwent IOL calculaton power determination using Haigis L software on the IOLMaster500 prior to cataract surgery. All eyes underwent cataract surgery by phacoemulsification MICS 1.8mm technique using the Stellaris platform. The preoperatvie plan was targeted for a slightly myopic outcome postoperatively between -0.25D to -0.50D to avoid any unwanted hyperopic surprise.

Results:

Statistical analysis revealed 58% of eyes achieved a postop outcome within +/- 0.25D of planned targeted refraction and 83% within +/-0.50D planned predictability. Only one eye ended up +0.50D hyeropic but still within the desireable +/-0.50D predictability range. The overall mean deviation was -0.11D+/-0.35D for all eye studies.

Conclusions:

IOL power calculations represent the largest malpractice risk to a practicing ophthalmologist in a 10 year study conducted by OMIC. Patients who have undergone prior LASIK have very high expectations at the time of cataract surgery, thus the need for precise IOL power calculations becomes even that more important. The IOL calculation methods most commonly used post LASIK include the Clinical History Method, the Contact Lens Method, and/or the ASCRS calculator using Masket, Shammas, or other formulae. The Haigis L solution has the advantage of not needing clinical history information pre-LASIK, not needing CL over-refractions, or time-consuming data entry into a website and analyzing multiple IOL power choices from various formulae. When using Haigis L, the only information needed is whether the patient had prior myopic or hyperopic LASIK. Haigis L avoids using corneal power readings to predict effective lens position (ELP). Haigis L is not recommended in post RK eyes. The analyzed data in this study shows the accuracy of postoperative outcomes compared to targeted refraction in post LASIK patients, making Haigis L the real solution in these challenging cases.
