Purpose:
To evaluate the effect of Nd:YAG capsulotomy treatment on anterior chamber depth(ACD) for the treatment of posterior capsular opacification(PCO) in 1-piece Hydrophilic acrylic IOLs (Akreos MI 60 and Akreos Adapt AO, Bausch&Lomb), by Laser Interferometric Biometry (IOL Master)

Methods:
Our prospective study included ten eyes of seven patients with posterior capsular opacification after uncomplicated phacoemulsification surgery with complete CCC and in-the-bag IOL implantation. The posterior capsulotomies were performed with a Nd:YAG laser (ZEISS, Visulas YAG III) and Abraham capsulotomy lens. ACD evaluation was performed by non-contact technique using Carl Zeiss IOL master 500 (version 7) immediate prior to capsulotomy, at 1 hour and one week after the procedure.
Results:
The mean ACD before Nd:YAG capsulotomy, in the first hour and one week after intervention were 4.72 ± 0.33 mm, 4.47 ± 0.5 mm and 4.21 ± 0.63 respectively. There were three patients lost to follow-up in the first week. At one week post procedure, the progressive anterior shifting of IOLs was positive in six eyes. Whereas posterior shifting was found in three eyes with less than 0.04 mm, one eye with 0.11 mm. IOLs position was steady in only one eye. At one week post capsulotomy, the absolute amount of ACD changes were forward IOLs movement in 12 eyes out of 14 eyes (mean -0.25 mm; range 0.41 to -1.54 mm).
Conclusions:
Conclusion : In contrast to previous studies, which utilized a different types of IOLs, reported no statistically significant change in IOLs position or backward IOLs movement (Oliver Findl[1999]), our studies found that capsulotomy of the 1-piece four haptics foldable hydrophilic acrylic IOL (Akreos MI60 and Akreos Adapt AO, Bausch&Lomb) caused a forward movement in most of the eyes. Interestingly, there were more myopic shift in the first week after the procedure compared to one hour post-Nd:Yag capsulotomy (mean difference= -0.17 mm; SD= 0.51). Further studies on PCO management after in-the-bag IOLs implantation need to be undertaken.
