Purpose:
To evaluate and compare the accuracy of different IOL calculation formulas / methods in eyes with high myopia [axial length (AL) > 26 mm]
Setting:
Ein-Tal Eye Center, Tel-Aviv, Israel
Methods:
Post-operative refractive results of 95 myopic eyes (AL>26.0 mm) were compared with the predicted refraction using 9 different formulas: 3rd generation 2 variables formulas (SRK/T, Holladay I, Hoffer Q) ; New generation formulas (Haigis, Barrett Universal formula, Holladay II, Olsen, Olsen-C (using the C constant) and The Koch & Wang AL adjustment method). The ULIB constants were used for all formulas, manufacturer constants were used for the AL adjustment method and an A-constant of 120.5 was used for the MA60MA lenses for the Barrett Universal formula
Results:
For IOLs with dioptric power higher than 5.0 diopters (D) a Mean Absolute Error (MAE) ? 0.5D was achieved in more than 75% of eyes using the AL adjustment for Holladay1 (91.4%), AL adjustment for SRK/T (85.7%), Barrett Universal formula (85.7%), SRK/T (82.9%), Haigis (80%), AL adjustment for Hoffer Q (80%) and Olsen (77.1%). For IOLs with dioptric power lower than 5.0 D a MAE ? 0.5D was achieved in more than 75% of patients using the Barrett Universal formula (84.6%) and the AL adjustment for Haigis (76.9%).
Conclusions:
IOL power calculation for eyes with high myopia requires axial lens adjustment and specific formulas to achieve a desired predictable postoperative refraction
