Purpose:
to evaluate the optical, functional outcomes, contrast sensitivity, optical phenomena and spectacle independence after implantation of multifocal IOL with variable refractive index design (gradient IOL).
Setting:
S.Fyodorov Eye Microsurgery Complex State Institution, Russia
Methods:
Nonrandomized prospective study included 42 eyes (36 patients). IOL optical part is composed of one polymer with different refraction index. Power difference between components - 3.5 diopters. The new generation multifocal IOL was transformed into C-loop design with 2 haptic elements for better refractive stability. Overall IOL diameter was changed to 12.0 mm. Mean age of patients was 69 years (range, 52-76). We evaluated uncorrected ?nd the best spectacle corrected visual acuity (VA) for far, near and intermediate distances, spherical equivalent (SE), contrast sensitivity (CS) and amplitude of pseudoaccommodation. The spectacle independence, glare, halos were assessed with a validated questionnaire
Results:
Three months after implantation mean uncorrected distance VA was 0.78, mean uncorrected near VA 0.63, mean uncorrected intermediate VA 0.58, mean corrected distance VA was 0.87, mean corrected near VA 0.76, mean corrected intermediate VA 0.72. Mean SE was -0.52 D, mean CS was 42.2. Mean amplitude of pseudoaccommodation was 4,53 diopters. 86% patients with multifocal gradient IOL did not need glasses for daily activities, driving and reading. Questionnaire revealed optical phenomena in post-op in 2 eyes
Conclusions:
The study found high functional results and quality of vision after implantation of multifocal IOLs with variable refractive index design (gradient IOLs), high spectacle independence and low incidence of optical phenomena
