Purpose:
Trifocal intraocular lenses (IOL) have been developed to fill the gap of limited intermediate vision as known for bifocal diffractive IOLs. These lenses provide an intermediate focus additionally to the near and distance focal points. With our study we want to show the impact of the third focal point on visual acuity, depth of focus and photic phenomena.

Methods:
In this prospective clinical study 120 patients underwent cataract surgery or clear lens extraction with implantation of a trifocal IOL (AT.LISA tri 839M). Three months after surgery best-corrected monocular visual acuity (CDVA) was measured and binocular uncorrected visual acuity was assessed at near (UNVA), intermediate (UIVA) and far distance (UDVA). The occurrence of photic phenomena was documented. Defocus curves were generated from -5 to +5 diopters in 0.5 D steps.
Results:
Three months postoperatively mean monocular CDVA was -0.01 logMAR with a mean subjective refraction of SEQ +0.15±0.1 D. The patients reached a binocular UDVA of 0.01, binocular UIVA of 0.1 and binocular UNVA of 0.1 (all logMAR). The defocus curves showed the typical pattern of trifocal IOLs with very similar peaks for near and intermediate vision. Mild glare and halos were reported by 18% and 15% of the patients, respectively. None of the patients reported any severe photic phenomena.
Conclusions:
The AT.LISA tri 839MP provided excellent visual acuity at all evaluated distances with a low occurrence of photic phenomena. The defocus curves showed a significant contribution of the third focal point to intermediate vision without compromising near vision.
