Purpose:
Trifocal design represents the last refinement in diffractive multifocal IOL. It aims an improvement in the intermediate vision but a decrease in quality of vision could be dreaded. We studied visual performances acheived after trifocal diffractive implantation.

University Hospital MORVAN - Brest- France
Methods:
The study consisted in a prospective monocenter randomized study of 22 bilateral implantation of the diffractive aspheric trifocal IOls AT Lisa tri - Zeiss. Patients were having a standard follow-up after multifocal implantation, where the monocular and binocular visual acuity was measured at 35 and 65 cm and 4 m under photopic and mesopic conditions. Evaluation of Quality of vision included questionnaire of life, contrast sensitivity, optical aberrations and scatterlight measurement.
Results:
The mean distance postoperative uncorrected visual acuity in LogMAR was 0.08±0.2. The mean intermediate and near visual acuity scored respectively LogMAR 0.09±0.10 and LogMAR 0.08±0.12. The defocus curve shows no decrease in the intermediate. The questionnaire analysis revealed a high patient satisfaction. The distance visual acuity showed slight decrease in mesopic conditions. The visual acuity under 25% and 10% contrast decreased in near and intermediate distances. Mean Induced high order aberrations was inferior to 0.3 microns
Conclusions:
The concept of diffractive trifocality appear to provide good vision at any distance with no subjective visual disturbance. Longer follow should evaluate the PCO occurrence and impact as well as the stability of outcome with the aging process.
