Abstract Details

Purpose:

To evaluate visual and refractive outcomes at postoperative year 1 in patients implanted bilaterally with 1 of 3 types of presbyopia-correcting intraocular lenses (IOLs): the Crystalens AO IOL, the ReSTOR +3 multifocal IOL, and the Tecnis Multifocal IOL.

Asian Eye Institute, Philippines

Methods:

This prospective, randomized, patient-masked study enrolled patients age 40 or older with less than 1.25 D of corneal astigmatism who were eligible for routine cataract surgery. Patients were randomized to bilateral implantation with accommodative Crystalens AO lenses, ReSTOR +3 multifocal lenses, or Tecnis Multifocal lenses during phacoemulsification. Patients were followed through postoperative month 12. The primary outcome measures included refraction (manifest refraction spherical equivalent, MRSE) and binocular high- and low-contrast uncorrected distance visual acuity (UCDVA), uncorrected intermediate visual acuity (UCIVA), uncorrected near visual acuity (UCNVA), and contrast sensitivity.

Results:

A total of 78 patients (156 eyes) were enrolled. At postoperative year 1, the mean MRSE was ?0.34 ± 0.36 D for Crystalens AO, +0.04 ± 0.41 D for ReSTOR +3, and +0.09 ± 0.47 D for Tecnis MF. Mean high-contrast visual acuity was: 20/20 for UCDVA, 20/20 for UCIVA, and 20/25 for UCNVA in the Crystalens AO group; 20/20 for UCDVA, 20/25 for UCIVA, and 20/20 for UCNVA in the ReSTOR +3 group; and 20/20 for UCDVA, 20/25 for UCIVA, and 20/20 for UCNVA in the Tecnis MF group. Mean low-contrast visual acuity was: 20/30 for UCDVA, 20/30 for UCIVA, and 20/40 for UCNVA in the Crystalens AO group; 20/30 for UCDVA, 20/40 for UCIVA, and 20/40 for UCNVA in the ReSTOR +3 group; and 20/30 for UCDVA, 20/40 for UCIVA, and 20/40 for UCNVA in the Tecnis MF group. Crystalens AO demonstrated better contrast sensitivity with glare than ReSTOR +3 at 6 cpd (p=0.02) and 12 cpd (p=0.03) and than Tecnis MF at 12 cpd (p=0.05).

Conclusions:

Long-term follow-up at postoperative year 1 in this study showed that uncorrected distance vision was similar among the groups. Uncorrected intermediate vision was better with Crystalens AO than with the multifocal ReSTOR +3 and Tecnis MF lenses under both high-contrast and low-contrast conditions. Uncorrected near vision was better with the multifocal ReSTOR +3 and Tecnis MF lenses than with Crystalens AO under high-contrast conditions but was similar among lenses under low-contrast conditions. Vision quality (contrast sensitivity) was better with Crystalens AO than with the multifocal ReSTOR +3 and Tecnis MF lenses.
