Corneal epithelial mapping with high-resolution OCT in keratoconus

Abstract Details

Purpose:

The high resolution OCT can measure the corneal epithelium with a ± 2µm precision. Several authors have described epithelium disorders in keratoconus patients with an importance for screening "forme fruste" keratoconus. The aim was to establish a relationship between the epithelial thickness and the anatomy of the cone.

Setting:

Clinic Monticelli, 88 rue du Cdt Rolland, 13008 Marseille, France

Methods:

100 frank keratoconus’ were studied. An isolated analysis is carried out on a few "forme fruste" keratoconus. The OCT scanning zone is 6mm. The parameters retained were: minimum corneal pachymetry, minimum epithelial thickness, maximum epithelial thickness, the difference between epithelial thicknesses (? mini – max), and superior - inferior variations (? S – I), the steepest keratometry (KS), mean keratometry (KM)

Results:

It appears that the epithelium thickness, the differences of epithelium thicknesses (? mini – max), (? S – I) are closely linked to the minimum corneal thickness, the steepest keratometry and the mean keratometry. However, we are not sure of a significant relationship for the "forme fruste" keratoconus

Conclusions:

Keratoconus severity (thinning and steepening of the cornea) is closely linked to the epithelial variations: minimum thickness, differences of thicknesses (min – max) superior - inferior variations
