Abstract Details

Purpose:

To determine visual outcome, reoperation rate and secondary glaucoma rate in eyes operated on in the first year of life with primary IOL implantation.

Setting:

Eye Hospital, University Clinical Center Ljubljana, Slovenia

Methods:

10 eyes of 8 babies operated during the years 2001-2003 and 15 eyes of 10 babies operated during the years 2006-2008 were included in the study. Basic surgical technique was similar in all eyes and consisted of anterior CCC, cataract aspiration, posterior CCC, anterior vitrectomy, in the bag IOL implantation and optic capture. All the records were evaluated and all children were thoroughly investigated. Mean follow-up of the first group (+10 group) was more than 10 years, whereas in the second group (+5 group) it was more than 5 years. Three parameters were evaluated in all eyes: BCVA, number of secondary procedures needed, presence or absence of secondary glaucoma.

Results:

In the +10 group 2 babies had bilateral cataracts and 6 had unilateral. Average postop. BCVA of the whole group was 0.4, in the eyes of babies with bilateral cataracts however it was significantly higher (0.7) compared to the eyes of babies with unilateral cataracts (0.2). The majority of eyes in this group didn’t need any secondary procedure, however in two eyes anterior synechiolysis was performed. Secondary glaucoma was present in 50% of eyes and was treated surgically in 2 eyes. In the +5 group 5 babies had bilateral cataracts and 5 had unilateral. Average postop. BCVA of the whole group was 0.4, in the eyes of babies with bilateral cataracts however it was higher (0.5) compared to the eyes of babies with unilateral cataracts (0.3). A secondary procedure was needed in one eye of this group. Secondary glaucoma was present in 40% of eyes and was treated surgically in 3 eyes.

Conclusions:

The results show comparable BCVA and reoperation rate between the groups, however secondary glaucoma rate increases with longer follow-up time.
