Purpose:
To create a less traumatic technology of cataract extraction after posterior chamber phakic IOL implantation.

Methods:
Bylensectomy was performed in 24 patients aged from 26 to 55 years (13 with initial myopia, 11 with hyperopia). All patients underwent conventional examinations for refractive patients, ultrasound bio-microscopy (UBM), ophthalmic ergonomic investigations, dopplerography. Traditional surgical technology was used in 6 patients (PIOL removal, cataract phacoemulsification, implantation of posterior chamber aphakic IOL). All stages of phaco, implantation of aphakic IOL, including capsular tension rings also were carried out before PIOL removal in 18 patients.
Results:
Visual functions after bilensectomy were 20/40 20/20 in all patients, it corresponded to the visual level after PIOL implantation. A tendency to a faster recovery of maximum possible visual functions was noted in patients operated according to the offered technology. In the same group of patients the loss of endothelial cell density was half compared with the group of traditional approach in follow-up of 1 year.
Conclusions:
The suggested technique of bilensectomy is a method of choice in surgical intervention decreasing a risk of intra- and post-operative complications, minimizing the surgical incision, IOP deviations and vitreous body shift.
