Abstract Details

Purpose:

To compare the outcomes of iris-fixation technique with a three piece foldable posterior chamber intraocular lens (IOL) and reverse iris claw lens implantation technique for the treatment of aphakia.

Setting:

Beyo?lu Eye Research & Training Hospital, Istanbul

Methods:

Forty-four eyes of 44 patients who underwent iris-fixated foldable posterior chamber IOL implantation with PC-9 prolene suture (Group 1) and 24 eyes of 24 patients who underwent reverse iris-claw IOL implantation (Group 2), were enrolled into this retrospective, comparative study. The patients with less than 3 months follow-up were excluded. Main outcome measures were change in visual acuity (VA); intraoperative, early and late postoperative complications.

Results:

The mean age of patients was 53,7±22,6 years in Group 1 and 62,4±20,8 years in Group 2. The diagnosis were 15 aphakia, 14 IOL dislocation and 15 lens subluxation in Group 1; 17 aphakia and 7 lens subluxation in Group 2. There were no intraoperative complications except minor hemorrages in both two groups. The best corrected VA improved from 0,12±0,17 to 0,38±0,31 (<0,001) in Group 1; from 0,22±0,19 to 0,39±0,25 (<0,001) in Group 2. The change in VA was similar between groups (p=0,21). The early postoperative complications were corneal edema in 6, positive Seidel test in 2, hyphema in 3, IOP elevation in 2 eyes in Group 1; corneal edema in 1, IOP elevation in 1 and severe anterior chamber reaction in 5 eyes in Group 2. The late postoperative complications were IOL decentralisation in 1 and glaucoma in 1 eye in Group 1; glaucoma in 1, IOL decentralisation in 2, retinal detachment in 1 eye and cystoid macular edema in 1 eye in Group 2.

Conclusions:

The iris-fixation technique with a foldable three-piece posterior chamber IOL and reverse iris claw lens implantation technique are safe and have similar clinical outcomes.
