Purpose:
Clinical evaluation into the safety and efficacy of performing refractive lens exchange procedures with the femtosecond laser refractive cataract surgery technique.

International Vision Correction Research Centre (IVCRC), Department of Ophthalmology, University of Heidelberg, Heidelberg, Germany
Methods:
In this single centre evaluation, the femtosecond laser was used to perform anteriorcapsulotomy and lens fragmentation in patients presenting for refractive lens exchange surgery. All procedures were performed with the Victus femtosecond laser platform (Bausch+Lomb/Technolas Perfect Vision) and different premium IOLs (multifocal, toric, aspheric) were implanted. Measured outcomes at one day as well as one and three months postoperatively includedaccuracy of the capsulotomy,phaco energy needed, visual outcomes and overall rating of the procedure.
Results:
To date, the femtosecond laser cataract technique has been used in 26refractive lens exchange cases. All surgeries have been uneventful and capsulotomieshave been well centred, precise and reliable. A perfect overlapping with a capsulotomy diameter of 1mm less than the IOL optic diameter was achieved for all IOLs implanted. All cases achievedgood visual outcomes with a 0.1 logMARmedian UDVA and 0.0 logMARmedian CDVA at three months postoperatively. Patient satisfaction was rated very high. The difference between target and achieved refraction was 0.04 D at one month and -0.14 D at three months postoperatively. No surgical complications were noted.
Conclusions:
Initial results using laser refractive cataract surgery find it can be safely and effectively used in cases of refractive lens exchange surgery. Especially in cases with clear lenses and a purely refractive surgery procedure the use of the femtosecond laser can further enhance the safety and quality of our surgical procedures.
