Purpose:
Cataract surgery with the femtosecond laser is a fast and delicate procedure, requiring high precision. The soft docking adopted does not prevent small patient / eye movement, with potential problems in procedure efficacy and safety. Real time OCT control may be of advantage, allowing the surgeon to monitor the procedure and to stop it should a problem occur.

Methods:
The first 50 eyes operated for cataracts with the VICTUS femtosecond laser were recorded and analysed for intra-operative complications and inconveniences of any kind. The value of real time OCT monitoring was assessed in terms of docking and procedure control, and in terms of surgeon comfort.
Results:
Real time OCT allowed the surgeon to closely monitor the procedure, and to compare the OCT images with the camera images. In one eye a small fluid bubble was noted before surgery between the corneal surface and the patient interface, suggesting re-docking. The peripheral fluid meniscus showed changes in two eyes during the procedure, and was corrected by lateral shifting of the bed. No other inconveniences were observed in this series of eyes.
Conclusions:
Real time OCT is very helpful during femtosecond laser cataract surgery. As the procedure progresses, any change in docking and any problem in laser action can be promptly detected and evaluated. This may result in some adjustment in patient position, or it may suggest procedure termination. The real time awareness of the femtolaser surgery outcome increases surgeons comfort.
