Purpose:
To investigate the possibility of reversing previous myopic correction after ReLEx smile (small incision lenticule extraction) and performing LASIK following the myopic reversal, as a method of creating monovision for presbyopic correction
Setting:
Tertiary Care Hospital
Methods:
ReLEx smile was performed on 6 rabbit eyes to induce a refractive spherical correction of -6.00D. The extracted lenticules were stored at -80°C for 14 days and autologous reimplantation was performed. Five weeks later, LASIK (-5.00D correction) was performed on 3 of these eyes (LR). The other 3 eyes were used as ReLEx smile re-implantation group (RO). LASIK alone to induce a -6.00D refractive spherical correction was performed on 3 additional eyes (LO). All rabbits were sacrificed 1 day post-operatively. Contralateral eyes were used as controls.
Results:
Increased light reflectance and acellularity was noted on in vivo confocal microscopy in the LR and LO groups relative to the control and RO groups. No statistically significant difference in reflectivity levels between LR and LO groups was observed (p=0.627). Corneal thickening, most likely due to oedema, was noted on AS-OCT. Immunohistochemistry demonstrated no significant differences in CD11b (p=0.219) and TUNEL (p=0.544) positive cells between LR and LO groups in the central cornea. In addition, fibronectin-staining intensity was similar between both groups
Conclusions:
LASIK following lenticule reimplantation induced similar inflammatory and wound healing responses as the corneas undergoing only LASIK. This technique appears to be feasible in the management of presbyopic patients who wish to undergo monovision correction following lenticule re-implantation
