Purpose:
Demonstration of successful management of a case of spherophakia with anterior subluxation and corneal endothelial adhesion and a pre operative best corrected visual acuity of Finger Counting at 3 metres.

B B Eye Foundation, Kolkata, India
Methods:
The spherophakic lens underwent complete separation from the corneal endothelium then continuous curvilinear capsulorhexis was done followed by phaco emulsification and in the bag implantation of a single piece lens. The lens was stable in the early post operative period but three months later there was anterior subluxation of the IOL bag complex. Fixation and proper stability of the IOL bag complex was done as a second surgery. The second eye of the same patient was taken up and after IOL implantation in the bag, the bag was stabilized by Cionni segments. One year follow up showed good stability of both the IOLs.
Results:
The patient achieved 20/40 vision in both eyes and the vision is maintained one year post surgery.
Conclusions:
Judicious use of capsular support devices, viscoelastics and gentle and patient approach can successfully address the challenge of Spherophakia
