Abstract Details

Purpose:

A prospective analysis of 196 cataract surgery done in Vitrectomized eye to evaluate & record the pre-operative and post-operative ocular behavior.

Setting:

Cataract surgery in vitrectomized eye has always remained an unsettling situation for the surgeon due to the complexity in surgical behavior and unpredictable outcome.

Methods:

Total 196 cataract surgeries were enrolled in the study between January 2003 and December 2011 where vitrectomy was done, but no silicone oil or gas remained in vitreous cavity. Clear corneal 2.8 mm Phacoemulsification was done in all the cases. A detailed note was taken in the operation and post-operative sheet regarding different unusual pre-op ocular behavior. Visual acuity was tested in four groups for pre-operative & post-operative records, & they were (A) <20/200, (B) 20/200 to 20/80, (C) 20/60 to 20/40 and (D) 20/30 or better. None of the patients received any medication other than the routine peri-operative regimen.

Results:

Disproportionate nucleus sclerosis (64) was the commonest unusual observation followed by less-dilatability of pupil (31), Floppy Iris (23), posterior capsular problem (21) & Zonular weakness / dehiscence (7). But, most encouragingly, 68 out of 196 operations did not show any unexpected behavior and were very smooth. Quantitative visual improvement was noted in all the four groups. In Group A, pre-op 165 patients slumped down to 23 (post-op) 7& in Group B 31 (pre-op) swelled upto 48 (post-op). None of the patients was in Group C & D pre-operatively, but post-operatively Group C had 79 patients & Group D had 46 patients. 13 patients in Group A had irreversible macular impairment.

Conclusions:

The study concluded that a good percentage of post-vitrectomy cataracts can be operated without any hurdle & post-op visual recovery is generally encouraging.
