Abstract Details

Purpose:

To study the results of manual SICS under topical anaesthesia for Mature and Brunescent Cataracts

Setting:

Vishnu eye clinic - A secondary eye care referral centre in a South Indian metropolis

Methods:

53 patients with Mature, Hypermature, Morgagnian and Dense Brunescent cataracts were enrolled in the study between Sep 2011 and Feb 2013. These patients were poor candidates for Phacoemulsification but were keen to have surgery done without an injection. Topical anaesthetic was proparacaine supplemented with subconjunctival lignocaine. No intracameral lignocaine was used. A superior 7.5 to 8mm tunnel was constructed and capsulorrhexis made. Nucleus delivery was done with an irrigating vectis and a 6.5mm IOL was implanted in the bag. Patients experience including pain perception was noted both subjectively and objectively. The postoperative astigmatism after 1month was studied. The other parameters recorded were corneal clarity and cellular reaction in the immediate post operative period.

Results:

Patients had minimal or no pain with topical SICS even in crucial steps. The Surgeon"s comfort level was favorable. The visual outcome was comparable to Phaco

Conclusions:

Topical SICS is safe and comfortable to be performed even in challenging cataracts. The Induced astigmatism is a concern.
