Purpose:
To retrospectively evaluate the clinical outcomes of aspheric corneal wavefront ablation profiles in LASEK and Transepithelial PRK treatments over a large population and long follow-up times.
Setting:
Dr.Massimo Camellin, SEKAL Rovigo Microsurgery Centre, Rovigo, Italy.
Methods:
Six hundred eyes treated by corneal wavefront LASEK and Trans Prk were retrospectively analyzed at 1 to 10 years follow-up. Custom Ablation Manager (CAM) software was used to plan wavefront-customized aspheric treatments, and the ESIRIS or AMARIS systems were used to perform ablations (SCHWIND eye-tech-solutions). In all cases, standard examinations including preoperative and postoperative wavefront analysis with a CW topographer (Optikon Keratron Scout) were performed. Clinical outcomes were evaluated in terms of predictability, refractive outcome, safety, and wavefront aberration.
Results:
At the last postoperative follow-up, 29% of eyes achieved uncorrected visual acuity 20/20 or better. Average defocus was reduced from -0.5+-3.0 diopters (D) preoperatively (range: -12.00 to +7.375 D) to +0.5+-1.9 D postoperatively. Astigmatism was reduced from 2.3+-1.9 D (range: 0.00 to 8.00 D) to 1.2+-1.7 D. Best spectacle-corrected visual acuity (logMAR) improved from +0.2+-1.2 to +0.1+-0.3 (P=.003). The treatment strongly reduced RMS HOA from 2,3+-1,5 to 0,6+-0,8 (p<.0001), but as well coma from 0.7+-0.6 ?m to 0.5+-0.5 ?m (P<.0001) and spherical aberration from 0.3+-0.3 ?m to 0.1+-0.3 ?m (P<.0001).
Conclusions:
These retrospective results indicate that the aspheric corneal wavefront customized CAM approach for planning ablation volumes is safe and effective. The CW customized approach shows its strength in cases where abnormal optical systems are expected. Apart from the risk of additional ablation of corneal tissue, systematic wavefront-customized corneal ablation can be considered as a safe and beneficial method.
