Purpose:
To highlight use of spectral domain OCT in analyzing inadvertent filtering bleb post manual small incision cataract surgery and subtle features of hypotony maculopathy
Setting:
Lotus eye care hospital, Salem, TamilNadu, India
Methods:
53 year old underwent manual small incision cataract surgery 2 years back presented with inadvertent filtering bleb and hypotony maculopathy. Two and half months post operatively, he was diagnosed to have asymptomatic conjunctival cyst and was managed conservatively. 5 line raster anterior segment OCT showed fistulous track communicating with subconjunctival filtering bleb and SD-OCT showed cystoid macular edema with serous neurosensory detachment. 3D topographic map showed wrinkling of ILM-RPE surface. Surgical bleb repair to achieve water tight closure was done.
Results:
normal intraocular pressure and foveal contour was restored, but there was no improvement in visual acuity. Surface wrinkles on 3D topographic ILM-RPE map although reduced but still persisted.
Conclusions:
Any conjunctival mass in the vicinity of sclerocorneal tunnel should be looked with suspicion as timely intervention may prevent irreversible damage caused by cystoid changes in neurosensory retina from setting in. Apparent reversal of foveal contour is a gross parameter. 3 D topographic surface appearance, its quantification and correlation with visual acuity needs further exploration.
