Purpose:

Comparison of efficacy of prednisolone 1%, dexamethasone 0.1% and difluprednate 0.05% to control inflammation after phacoemulsification and to compare their adverse effects

Setting:

Vardhaman Mahavir Medical College and Safdarjung Hospital, New Delhi, India( tertiary centre in India)

Methods:

After unilateral uneventful phacoemulsification cataract surgery patients underwent anterior segment evaluation by slit lamp examination, IOP by non contact tonometry, subjective evaluation of local side effects on day 1, 3,7,14 and35.Paired t test was applied for comparison of data within a three drug groups, and for comparing between the between surgical groups, analysis of variance test was applied.

Results:

Of 90 patients, 30 patients received prednisolone 1%, 30 patients received dexamethasone 0.1% andv30 patients received difluprednate 0.05%. The mean anterior chamber (AC) cells loss on day 7 as compared to day 1 was maximum in difluprednate group 2.967, 2.833 for dexamethasone, 2.533 for prednisolone. While anterior chamber flare loss on day 3 as compared to day 1 was 1.200 in difluprednate group , 1.200 for dexamethasone, 1.333 for prednisolone. However on comparing three groups p =0.493 for AC cells and p= 0.168 (p > 0.05) for AC flare which is not significant. No IOP spikes were noted in measured IOP of any patient during duration of study. The adverse effects reported were few mild discomfort, foreign body sensation and watering.

Conclusions:

The study enrolled 90 eyes of 90 patients. all three drugs prednisolone 1%, dexamethasone 0.1% and difluprednate 0.05% were seen to have equivalent efficacy to control post operative inflammation with some difference in rapidity to do so difluprednate and prednisolone being more rapid compared to prednisolone.No serious adverse effects were seen during the study
