Purpose:
To evaluate the clinical outcomes with a new rotationally asymmetric multifocal toric intraocular lens (IOL) implanted after phacoemulsification in patients with pre-operative corneal astigmatism.
Setting:
Optegra Eye Sciences, Optegra UK.
Methods:
Observational case series. Forty nine eyes (28 patients) with ? 0.75 diopters (D) of corneal astigmatism underwent phacoemulsification and Lentis MPlus MF-15 toric IOL implantation by a single surgeon at two sites. Preoperative corneal astigmatism was determined using the OCULUS Pentacam and Zeiss IOLMaster. Pre- and post-operative monocular uncorrected and corrected distance (UDVA and CDVA) and near (UNVA and CNVA) vision was recorded along with manifest refraction, keratometry and operative and post-operative complication rates. Patient satisfaction, spectacle independence and symptomatology were assessed from inspection of electronic case notes.
Results:
Patients ranged in age from 47 to 76 years (mean±SD 61±7 years). Monocular pre-operative and post-operative UDVA was LogMAR 0.78±0.39 and 0.06±0.18 respectively (p<0.05). Pre-operative and post-operative CDVA was 0.10±0.22 and 0.06±0.18 respectively (p>0.05). Mean post-operative UNVA was LogMAR 0.37±0.21.92% of eyes achieved UDVA 6/12 or better post-operatively, 76% of eyes achieved UDVA 6/7.5 or better and 63% of eyes achieved 6/6 and N6 in combination. CDVA was 6/12 or better in 100% of eyes and 6/6 or better in 82% of eyes. The mean refractive cylinder was 1.37±0.98 D preoperatively reducing to 0.41±0.40 D postoperatively (p<0.05). 88% of eyes had residual refractive astigmatism of ?0.75D. Vector analysis of attempted versus achieved correction showed 92% of eyes within ± 1.00 D Error Vector (EV) and 73% within ± 0.50 D EV. There were 0 operative complications recorded and 2 post-operative complications (4%). 93% of patients reported they were satisfied with their vision post-operatively, 71% reported being spectacle independent for everyday tasks and there was a low incidence of haloes/glare reported by patients (4%).
Conclusions:
The Lentis Mplus toric MF-15 IOL is an effective way to enhance both distance and near vision following phacoemulsification in patients with corneal astigmatism.
