Abstract Details

Purpose:

To measure and evaluate the changes in ocular wavefront aberrations, for far, intermediate and near vision after WIOL –CF accommodative IOL implantation.

Setting:

Institute of Vision and Optics, Medical School,University of Crete, Greece

Methods:

15 subjects were enrolled in this prospective study. All patients had uneventful Cataract surgery with binocular implantation of WIOL-CF IOL (Medicem, Czech Republic). Zernike coefficients from second to fourth order were obtained 1 year postoperatively with natural pupil diameter and under mesopic conditions. The patient was asked to look at an illuminated target (letter E) at three different distances (far: 4m, intermediate: 60cm, near: 33cm). At the same time wavefront measurements were performed with i-Trace (Tracey Technologies, USA) wavefront analyzer.

Results:

Small changes in refraction were observed (mean -0.75 D+/-0.25) between the far and the near distance stimulus. For different distances, changes in higher order ocular aberrations where observed. Shift of spherical aberration to negative values were found as well as change of the vertical coma. These changes have been significant only between far and near stimulus. No significant changes of high order aberrations have been found between far and intermediate target distance. Spherical aberration changed in average by -0.18 +/-0.13?m between far and near and coma in y-axis by -0.23 +/-0.15 microns.

Conclusions:

The observed change in wavefront aberrations between near and far vision after implantation of WIOL-CF intraocular lens can justify the satisfactory visual outcome for near distance. Two mechanisms seem to be combined to provide a pseudo-accommodative effect. One is the small change in defocus and the other the overall change in HO aberrations resulting in an increase of depth of focus.
