Corneal biomechanical property changes after coaxial 2.2mm microincision cataract surgery and standard coaxial 3.0mm phacoemulsification

Abstract Details

Purpose:

To compare the changes in corneal biomechanics measured by Ocular Response Analyzer (ORA) after 2.2 mm coaxial microincision cataract surgery and 3.0 mm standard coaxial phacoemulsification.

Setting:

A tertiary level ophthalmology hospital. Taiyuan, China.

Methods:

The study comprised eyes with cataract that had 2.2 mm or 3.0 mm incision phacoemulsification. Corneal hysteresis (CH) and the corneal resistance factor correlated with intraocular pressure were measured by ORA.

Results:

In both groups, there was significant decrease in CH in the immediate postoperative period (P<0.05). In 2.2mm group, compared to preoperative value, there was no statistical difference at the end of the first week (P=0.249). In 3.0mm group, compared to preoperative value, there was still statistically difference at the end of the first week (P=0.009), there was no statistical difference at the end of 2 weeks (P=0.264). In both groups, the value of CRF was no statistical significant change after surgery. In both groups, mean IOPcc and IOPg increased 1 day postoperatively, and then decreased 1 week postoperatively. There was no statistical difference at the end of the first week (P=0.491, P=0.923) in 2.2mm group compared to preoperative value. But there was still statistically difference at the end of the first week (P=0.005, P=0.029), not until the end of 2 weeks (P=0.347, P=0.887) in 3.0mm group.

Conclusions:

Significant differences between preoperative and postoperative values were found for CH, CRF, IOPcc and IOPg. The 2.2mm coaxial micro-incision group seemed recovered quickly by the data of corneal biomechancis. Smaller incision made smaller changes of the cornea, resulting faster reconstruction of cornea postoperatively.
