Purpose:
To investigate the effect of an open bag IOL on LEC growth in serum and serum-free media in a human capsular bag model.

Methods:
A prototype IOL which maintains an open bag capsular bag configuration or a single piece Acrysof IOL were grown in match-paired human capsular bags in either serum-free or media supplemented with 2% human serum and 10ng/ml TGF beta 2. Cell growth was monitored with phase-contrast microscopy and at 28days capsular bags were harvested and analysed by immunocytochemistry for vimentin, F-actin and chromatin.
Results:
The open bag IOL showed a reduced rate of cell coverage of the posterior capsule compared to the single piece IOL in both culture conditions. This difference was more pronounced in the serum supplemented capsular bags.
Conclusions:
The presence of serum enhanced the shrink-wrapping of the capsular bag around the IOL which we propose creates and enhanced barrier to cell growth.
