Abstract Details

Purpose:

To report the outcomes of changes in straylight before and after phacoemulsification in eyes with visual acuity better than logMAR 0.1 (Decimal 0.8, Snellen 20/25).

Setting:

Private Refractive Surgery Clinic affiliated with an academic hospital

Methods:

A prospective case cohort of 161 eyes that underwent standard phacoemulsification procedures. Indication for surgery was either a cataract or a refractive lens exchange. Intraocular lenses used where either monofocal, or multifocal diffractive or refractive types. Parameters compared were: straylight values, CDVA, and refractive error. When available, VF-14 questionnaire results were evaluated.

Results:

In 161 eyes of 89 patients straylight was obtained prospectively before and after phacoemulsification. The mean age of the patients was 59.4 + 8.1 (range 44.5 to 83.8) years old. There were 51 females and 38 males. The mean preoperative UDVA was logMAR 0.42 + 0.41 (range 1.3 to -0.06). The mean postoperative UDVA was 0.04 + 0.09 (range 0.3 to -0.1). The mean preoperative CDVA was 0.02 + 0.05 (0.1 to -0.1) and postoperatively it was: 0.0 + 0.04 (0.2 to -0.1). Mean preoperative straylight was log (s) 1.21 + 0.2 (0.8 to 1.74), while the mean postoperative log(s) was 1.11 + 1.16 (0.76 to 1.63), this improvement was statistically significant. The mean difference was 0.08 log(s), which, like in the visual acuity LogMAR scale is logarithmic which can be compared to 4 letters of improvement on the acuity chart. In 44 eyes of 161 eyes the improvement of straylight was greater than log (s) 0.2, which is comparable to a 2 line improvement on the acuity chart. There was a correlation between preoperative high straylight values to postoperative improvement in straylight value.

Conclusions:

In eyes with relatively good CDVA of logMAR 0.1 or better (Decimal 0.8 or better, and Snellen 20/25 or better) straylight improved by 0.08 Log (s). There was a subgroup of 44 eyes, which reached an improvement of >0.2 log(s), which is comparable to a 2 line improvement on the vision chart. This result greatly supports the notion, that in patients with good visual acuity on reading charts, straylight measurements can help discern between those patients who will benefit from phacoemulsification from those who will have a lesser effect in terms of visual benefits.
