Purpose:
To compare torsional emulsification efficiency and anterior chamber stability of a new innovative fluidics system with an existing fluidics system
Setting:

Methods:
A new innovative fluidics management system that is able to maintain a certain target intraocular pressure during phacoemulsication procedures was evaluated. Anterior chamber stability was assessed during surgery at various vacuum settings. Aspiration flow was set at 25 ml/min.
Phacoemulsification efficiency was estimated from cumulative dissipated energy delivery numbers and personal impression.
The current phaco machine was set at the routinely used vacuum, 300 mm Hg, and 25 ml/min aspiration flow.
All surgeries were performed with the 0.9 minitip and torsional ultrasound.
Results:
During the initial evaluation, which could not include a prospective randomized comparative trial, the new fluidics system was used with vacuum settings up to 600 mm Hg. With a moderate target IOP of 40 mm Hg, anterior chamber stability was excellent. Very little indications of IOP variation, which are commonly observed with a traditional gravity fed irrigation line, were observed. Torsional emulsification seems significantly more efficient compared to the current system with vacuum set at 300 mm Hg. Preliminary results indicate a 30% lower energy delivery.
Conclusions:
A new phacoemulsification machine with an innovative fluidics system seems to provide superior emulsification efficiency and an enhanced anterior chamber stability.
