Purpose:
To evaluate clinical results after myopic or myopic astigmatic LASIK using an combined aspheric and wavefront-guided approach (Zyoptix Personalized Treatment Advanced) versus an purely aspheric software (Zyoptix Aspheric Treatment) for the Technolas 217 Z100 laser.
Setting:

Methods:
Observational single-center study. Inclusion criteria: myopic astigmatism ≤-10.0D with cylinder ≤5.0D, corrected distance visual acuity preop ≥0.8, mesopic pupil size >5.5mm.
Group I: aspheric wavefront-guided treated eyes (n=202), group II: aspheric treated eyes (n=161).
Measurements preop and 3 - 6 months postop: distance visual acuity (decimal scale), automated refraction (Canon R-F10), total higher order aberration (HOA) root mean square (RMS), spherical aberration (Zywave 5.2 Aberrometer, Technolas Perfect Vision), low contrast visual acuity (Sloan Letters Translucent Contrast Chart).
Results:
Mean corrected distance visual acuity preop, group I: 1.00, group II: 0.94. Mean uncorrected distance visual acuity postop, group I: 0.96, group II: 0.90. Mean spherical equivalent postop in automated refraction group I: -0.24 D (range -1.75 to +0.88 D), group II: -0.22 (range -2.25 to +0.63). Efficacy index, group I: 0.96, group II: 0.96. Mean HOA RMS, group I: preop 0.42 ±0.14µm, postop 0.46 ±0.23µm; group II: preop 0.37 ±0.17µm, postop 0.48 ±0.18µm. Mean spherical aberration (OSA notation), group I: preop 0.15 ±0.16µm (range: -0.22 to 0.59µm), postop 0.12 ±0.14µm (range: -0.44 to 0.56µm); group II: preop 0.15 ±0.19µm (range: -0.33 to 0.72µm), postop 0.12 ±0.21µm (range: -0.43 to 0.55µm). Mean low contrast visual acuity (logMAR), group I: spectacle corrected preop 0.29 ±0.12, uncorrected postop 0.31 ±0.13; group II: spectacle corrected preop 0.28 ±0.10, uncorrected postop 0.32 ±0.13.
Conclusions:
Combined aspheric wavefront-guided ablations yielded slightly better clinical results than purely aspheric ablations in myopic astigmatic corrections.
