Clinical significance of the keratometric difference between steepest corneal point and its symmetric site in keratoconus

Purpose:

To study the relationship between keratoconus severity and difference between the highest and lowest keratometry values.

Setting:

: Ophthalmology Department, Medical School of Yildirim Beyazit University, Ankara, Turkey.

Methods:

Sixty keratoconus cases were included in the study. Only one eye of each case was taken into the study. Cases were grouped into 4 according to four grades of Amsler Krumeich classification. There were 15 cases in each group. Keratometric and pachymetric measurements were recruited from Sirius dual Scheimpflug system. These measurements were studied: simulated keratometry, the highest keratometry value, keratometry value of the point symmetric to the highest keratometry value, corneal thickness.

Results:

Grade I keratoconus group had a mean value for keratometry difference between steepest point and its symmetric site as 13,39±5.03 D. These mean values for Grade II, Grade III and Grade IV groups were 15±5.60 D, 24±11.96D, and 23±14.01 D respectively. Kruskal-Wallis test showed that there was a statistically significant difference among these four groups (p=0.025). Although difference between Grade III and Grade I groups was significant according to the posthoc tests with Bonferoni correction, other two group comparisons failed to show any significant difference.

Conclusions:

The difference between the highest keratometry value and keratometry value of the point symmetric to this steepest point shows an increase in accordance to increase in keratoconus severity in early and moderate cases. On the other hand this increase does not continue in advanced cases, even a small drop occurs.
