Purpose:
To report the long term outcomes after WIOL – CF accommodative intraocular lens implantation
Setting:
Institute of Vision and Optics (BEMMO - IVO), University of Crete
Methods:
Twenty five patients (50 eyes) with mean age 65, 3 ± 8, 4 years (range from 53 to 83 years) underwent routine cataract surgery and WIOL – CF accommodative intraocular lens implantation. The WIOL – CF is inserted through a 2.8mm incision in its temporarily plasticized state. The mechanism for near focus of the WIOL – CF accommodative IOL depends on the lens deformation due to the action of natural focusing apparatus (cilliary muscle and zonules). The WIOL-CF curving during the near-focusing leads to an increase in the refractive power of the lens. This refractive power increase assists near vision (e.g., for reading). RESULTS Mean follow up was 14,32 ± 2,68 months (range from 12 to 24 months).Uncorrected Distance Visual Acuity (UDVA) improved from 0,31 ± 0,17 (range from Counting Fingers (CF) to 0,9) preoperatively to 0,77 ± 0,20 (range from 0,2 to 1) (decimal scale) and Corrected Distance Visual Acuity (CDVA) improved from 0,61 ± 0,19 (range from 0,2 to 1) preoperatively to 0,86 ± 0,14 (range from 0,5 to 1) (decimal scale) at the last follow – up examination. Eighty nine percent of patients gained one or more lines of CDVA while 75% of our patients had J1 to J1 – J2, at the last follow up, measured with Birkhauser reading charts at a distance of 35cm under photopic conditions. No complications occurred intra or postoperatively. Pseudo - accommodation was assessed using the iTrace technology and monitored by the use of an anterior segment Optical Coherence Tomography (AS - OCT).
Conclusions:
WIOL – CF accommodative intraocular lens implantation by accomplishing lens movement can restore some degree of natural accommodation providing satisfactory visual outcomes for both distant and near vision.
