Purpose:
To evaluate changes in intraocular pressure (IOP) and anterior chamber angle (ACA), anterior chamber depth (ACD) and anterior chamber volume (ACV) after phacoemulsification with intraocular lens (IOL) implantation. The relationship between the degree of IOP reduction and the variation of ACA, ACD and ACV were also studied.

Methods:
A prospective study including 49 patients selected for phacoemulsification with IOL implantation. Measurement of anterior chamber angle width, anterior chamber depth, anterior chamber volume and IOP using a Scheimpflug videophotography system and Goldmann’s applanation tonometry, respectively, were performed before and four weeks after surgery. Lens thickness was also recorded. RESULTS The mean age was 73,0 ± 8,7 years. The mean preoperative and postoperative IOP were16,55 ± 2,51 and 13,59 ± 2,25 mmHg, and this represented a mean decrease of 2,96 ± 1,79 mmHg (18%; p=0). After cataract surgery, the mean ACA, ACD, and ACV values increased by 13,63 ± 6,11º, 0,88 ± 0,38 mm, and 43,47 ± 22,21 mm3, respectively. The mean lens thickness was 4,67 ± 0,44 mm. Change in IOP was statistical significantly (p=0). IOP reduction was significantly positively related with angle widening (r= 0,296; p= 0,039), but not significantly positively related to increase in ACD (r= 0,135; p= 0,354) and to increase in ACV ( r= 0,114; p= 0,435). The angle widening was inversely related to preoperative ACA (r= -0,706; r2= 49,9%; p= 0). Preoperative ACD was significantly inversely related with the extent of ACD increase (r= -0,856; r2=73,3%; p=0). Increases in ACV was significantly inversely related with preoperative ACV (r= -0,86; r2=73,3%; p=0). Lens thickness was positively related to angle widening (r= 0,413; r2=17,1%; p=0,003), to the increase in ACD (r=0,608; r2= 36,9%; p=0) and to the increase in ACV (r= 0,55; r2 = 30,3%; p=0). IOP reduction was not related to lens thickness (p= 0,42).
Conclusions:
Phacoemulsification with IOL implantation results in significant mean reductions in IOP and concurrent increases in ACA width, ACD and ACV. It is expected that thick lens exhibited a greater increase in angle widening, anterior chamber depth, anterior chamber volume and a greater reduction in IOP after cataract surgery. However, no significant relation between lens thickness and surgically induced IOP reduction was observed.
