Abstract Details

Purpose:

The purpose - to spend clinical supervision over the patient with sharp postoperative bacterial endoftalmites, complicated the cataract surgery, an accompanied hypostasis and total infiltration of a cornea.

Setting:

Department of Ophthalmology, Eye Hospital T.I. Eroshevski, Samara, Russia

Methods:

Under supervision there was patient P, 75 years at which in 3 days after phacoemulsification with implantation IOL has developed sharp bacterial endoftalmites with total infiltration of a cornea. RESULTS By the first stage before planned endoocular intervention it is executed subtotal removing of a cornea with time closing of defect by an artificial cornea from PMMA. Then it is made subtotal 23-gauge 3-port vitrectomy with full removal changed vitreous and posterior hyaloids membranes. In process of vitrectomy sites toxic retina are revealed. All manipulations passed under the good visual control, thanks to an absolute transparency of an artificial cornea. After vitrectomy was done the artificial cornea is replaced by a donor corneal transplant. The postoperative period smooth. Visual acuity at an extract and in the remote terms - 0,1. The eye is quiet, the corneal transplant is almost transparent, in a retina dystrophic changes.

Conclusions:

Application of an artificial cornea creates possibility and good visual conditions for carrying out vitrectomy at sharp endoftalmites, accompanied total defeat of a cornea.
