Effect of preoperative and intraoperative factors on corneal endothelial cell loss after phacoemulsification

Abstract Details

Purpose:

To evaluate the effect on the corneal endothelial cell loss of preoperative (age, endothelial cell density, cataract density, anterior chamber dept and volume) and intraoperative factors (US energy, total phaco time ,effective phaco time and BSS consumed).

Setting:

Hospital Pedro Hispano, Matosinhos, Portugal

Methods:

Phacoemulsification was performed in 52 eyes of 51 patients with senile cataract (medium age of 75 +/- 7 years) without other ophthalmologic pathology , by the same surgeon using the same technique. Preoperative data were assessed 1 week before surgery: anterior chamber dept and volume using SIRIUS CSO® and cataract density classified according to LOCS III . Phaco time, US energy and BSS were noted during surgery. Endothelial cell density was measured using specular microscopy (TOMEY EM 3000®) before surgery and 1 day and 3 months after. Data were analyzed using SPSS 17.0. RESULTS The preoperative mean endothelial cell count was 2492 +/-240 cells/mm2 , being reduced to 2176 +/- 317 cells/mm2 after surgery (average reduction of 12,4%). We found a statistically significative association between endothelial cell loss after phacoemulsification and cataract density ( r=0,606 p<0,01) , BSS volume consumed (r=0,375 p=0,007) and US energy (r=0,345 p=0,016) However age, preoperative endothelial cell density, anterior chamber dept and volume, total phaco time and effective phaco time were not significant predictors of loss.

Conclusions:

There is a statistically significative relation between endothelial cell loss and cataract density, BSS volume consumed in surgery and US energy.
