Abstract Details

Purpose:

The purpose of the study was to assess the results of the cataract surgery with simultaneous astigmatism correction with the toric intraocular lens (t-PCIOL), and answer if rotation stability and changes in contrast sensitivity have influence on patients’ subjective satisfaction.

Setting:

Department of Ophthalmology, Silesian University of Medicine

Methods:

The study group consisted of patients,motivated to lower the need for glasses with the regular corneal astigmatism above 1,25 D. The research was based on 40 eyes of 28 patients (22 females and 6 males) at the average age of 59,85 9,4 years. In 31 patients the surgery was carried out in both eyes. The objective mean of refractive error was: spherical equivalent (SE) -1,68 ± 5,56 D, complete refractive astigmatism 2,74 ± 1,08 D, corneal astigmatism 2,52 ± 0,75 D, lenticular astigmatism 0,22 ± 0,79 D. T-flex Toric IOL (Rayner, Great Britain) was implemented: T-flex 623T model into 26 eyes and T-flex 573 T into 14 eyes. The mean spherical equivalent was 20,0 D, and the mean torus was 3,0 D on IOL plane. The average preoperative visual acuity was: UDVA 0,15; CDVA 0,55; UNVA 1,84; CNVA 0,5. The assessment measured: stability of the surgery results, efficacy, safety and surgery predictability, changes in contrast detection, intraoperative and patients' subjective satisfaction after the surgery. The contrast sensitivity was controlled after 1, 3, 6 and 12 months since the date of the surgery. PCIOL rotation angle was drawn after 6 months and the patient filled in an additional control satisfaction questionnaire RESULTS A statistically significant improvement of uncorrected and corrected distans and near visual acuity has been proven. Efficiency of the surgery: UDVA was 25 times better than CDVA before the operation. Safety of the surgery: only 1% of all eyes lost 1 line of CDVA during the post-op period. CDVA have not changed in 5,5% of all eyes, whereas the 93,5% gained 1 line. The largest CDVA improvement up to 9 lines, was noted in 4,5% eyes. The predictibility of the refractive astigmatism correction amounted from 85% after 1 day up to 95% after 1 month. The average T-flex rotation in the capsule after 6 months was 2˚. In 97,5% of cases the rotation became lower than 10˚. In 1 eye reposition of the IOL after one day from the first surgery was required. A statistically significant increase in contrast sensitivity on all spatial frequencies has been noted. The subjective analysis of sight functioning has revealed a high level of satisfaction. The level of difficulty in performing common daily activities, dependent on sight, has been reduced on average by 36,2% in patients evaluation. Glasses were now used on average by 48,1% less frequently

Conclusions:

The stability of the cataract surgery effects with the toric T-flex PCIOL implantation considering the ractive effect, postoperative visual acuity is high. IOL rotation stability, improvement in contrast sensivity and the level of patients subjective satisfaction is very high
