Follow-up results of combined phacoemulsification and non-penetrating deep sclerectomy in patients ill with open angle glaucoma

Purpose:

According to different authors, glaucoma accompanies cataract in 17-76% of cases. Methods which combine phacoemulsification (PE) and antiglaucoma operations are considered to be rather promising in modern ophthalmosurgery. The purpose of our work was to evaluate the effectiveness and safety of combined PE and nonpenetrating deep sclerectomy (NPDS).

Setting:

Khabarovsk branch of the academician S.N. Fyodorov Federal State Institution" Intersectorial Research and Technology Complex " Eye Microsurgery" .

Methods:

We observed 54 patients (60 eyes) who had a combination of complicated cataract and open angle glaucoma (OAG). Their age varied from 51 to 73 years. Preoperative BCVA varied from 0.01 to 0.5. Preoperative IOP was 24-31 mm Hg. The anterior chamber angle was open in all cases. All patients underwent combined surgery which consisted of PE with IOL implantation and NPDS with separated accesses.

Results:

All operations passed uneventfully. 5 patients had local cornea edema in early postoperative period, 1 patient had hyphema up to 1mm which resolved spontaneously on 3-5 postoperative day. Vision acuity increased in all patients on 2-3 postoperative day, and made 0.4-1.0. Vision acuity less than 0.7 was explained by previous macular pathology and/or glaucoma atrophy of optic nerve. IOP lowered in all cases and varied from 14 to 17 mm HG. Vision acuity remained stable in majority of cases one year later the performed operation. Vision acuity went down to 0.2-0.3 in 3 patients because of development of posterior capsule fibrosis which required YAG-laser discission. IOP remained normal (16-21 mm Hg) in 48 eyes in follow-up period without any hypotensive therapy; IOP remained stable on hypotensive therapy (bis in day instillations of 0.5% timolol and 2% dorsopt drops); and 2 eyes required recurrent antiglaucoma surgery because of IOP elevation.

Conclusions:

Combined operative treatment (PE with IOL implantation and NPDS) allows normalizing IOP and increasing vision acuity in majority of patients ill with combined pathology – cataract and glaucoma. FINANCIAL DISCLOSURE?: No
