Sulcoflex piggyback IOL as a refractive solution in the hyperopic patient

Purpose:

Purpose: To present the results of implanting a secondary intraocular lens, Sulcoflex, in two eyes in two patients. Patient A was excessive hyperopic and astigmatic (+9.25 -4.25 x 178) prior to the first IOL implantation (Acrysof Toric IOL), with a postoperative residual refractive error of +5.25 -2.50 x 45. Patient B, bilateral hyperopic, had cataract surgery in one eye 19 years ago with an intended postoperative refraction of +5.50 -1.25 x 84, now referred for cataract surgery in one eye and a refractive solution in the pseudophakic eye.

Methods:

Methods: Patient A received a Sulcoflex toric IOL (653T) and patient B an aspheric Sulcoflex IOL (653L). Refraction and visual outcome were evaluated at day one, 2 weeks and 4 months postoperative. Scheimpflug was performed.

Results:

Results: Both patients had uneventful surgery. Four months postoperative patient A presented with best corrected visual acuity 20/25 with a refraction of 0,0 -0,75 x 115 (slightly amblyopic) and patient B with best corrected visual acuity of 20/20 with a refraction of -0.75 -0.50 x 60.

Conclusions:

Conclusion: For an excessiv hyperopic and astigmatic patient, as patient A, there are only limited possibilities for an emmetropic solution. A two-step procedure will always be necessary. Secondary IOL implantation with Sulcoflex IOL provides a safe and predictable refractive outcome in patients where laser refractive surgery is not possible. And in general it is a safe end effective method to improve pseudophakic refractive outcome. FINANCIAL DISCLOSURE?: No
