Purpose:

To develop a Surgical tactics in treatment of cataract, complicated with decompensated neovascular glaucoma and diabetes mellitus.

Setting:

Ophthalmology Clinic " OKOMED" , Moscow, Russia.

Methods:

One of patients with multiple subconjunctival scleral trephinations at the area of ciliary body, pars plana and ecvatorial part of the retina, with subsecuent U.S.phaco + IOL and anti-VEGF treatement with comparison to the fellow eye of the same patient. Both eyes previously had cataracts, complicated with decompensated neovascular glaucoma, hyphema, corneal oedema and diabetes mellitus. The fellow eye was unsucsessefully treated with the well-known and wide - spreaded penetrative method

Results:

At the next day after non-penetrative anti-glaucoma operation we observed a normalizing of the IOP from 45 to 16 mm Hg., clear cornea, deepening of the anterior chamber with widening of its angle, absens of preoperatively present hyphema and neovascularisation. That took us a possibility for further succesfull phacoemulsification, IOL implantation and anti – VEGF intraocular treatement.Successfull outcome of surgical tactics with long - term compensation of the IOP, resolving of the hyphema, retinal haemorrage, oedema and improvement of visual acuity. Stable functions during post-operative period we can see for more than 2 years up to now.

Conclusions:

A multiple subconjunctival scleral trephinations at the area of ciliary body, pars plana and ecvatorial part of the retina, with subsecuent U.S.phaco + IOL and anti-VEGF treatement is the safe and effective surgical tactic for managing of the most dramatically difficult situations in oftalmosurgery.
