Purpose:
To assess the effect of age, gender, laterality and refraction on the keratoconus (KC) diagnostic indices with Pentacam and the need to adjust the normative values accordingly.
Setting:
National Eye Hospital, Cairo, Egypt.
Methods:
In a retrospective study, 101 eyes of normal persons without keratoconus were tested by slit lamp and Pentacam (distributed by WaveLight, Erlangen, Germany under the name of Allegro Oculyzer). Age, gender, laterality and subjective refraction were recorded. The effect of these factors on elevation and thickness based KC detection indices is studied by t-test and partial correlation test.
Results:
When we exclude the age effect, refractive spherical equivalent (SEq) was correlated to all tested indices except anterior elevations (AE) when 9 mm zone is analysed (P= .059). On the other hand, despite the exclusion of the SEq effect, still the age was correlated to AE, posterior elevations (PE) and difference between them (P-A E) for 8 then for 9 mm zones (P: <.001, <.001, .019, <.001, <.001 and <.001, respectively). On the contrary, it is not correlated to pachymetry progression indices (Pmin, Pavg, Pmax), thinnest point thickness (TPT) or apex-thinnest location distance (ATD) (P= .136, .945, .216, .220 and .301, respectively). Gender and laterality were not related to the values of KC indices tested.
Conclusions:
Some KC detection indices may be affected by age and SEq. This necessitates readjustment of normative values. The use of the more robust thickness related indices, rather than elevations, is recommended in extreme of ages.
