Purpose:
To assess the potential added value of topography-guided astigmatic laser vision correction with iris-registration (TLI) in real practise.
Setting:
National Eye Hospital, Cairo, Egypt
Methods:
In a prospective study, 49 eyes with preoperative astigmatism of 0.75 to 6.0 D were operated upon by TLI using the EX500 excimer laser and Topolyzer Vario topography (WaveLight, Erlangen, Germany). Preoperative subjective refraction and corneal topography were done. Intraoperative pupil centroid shift (PCS) and cyclotorsion angle (CTA) were recorded.
Results:
The preoperative astigmatism was -2.5 ± 0.1 D. Diagonal pupil decentration was 251.9 ± 126.3 ?m. The Diagonal intraoperative PCS compensation was 210.8 ± 105.5 ?m. The CTA alignment was 3.1 ± 2.1°. The calculated under-correction of astigmatic error would be 0.26 ± 0.20 D (0.03 to 0.93D) in these cases if not compensated for. The horizontal component of angle kappa was larger in cases with higher topographic astigmatism (P=.039). In turn, it was associated with larger intraoperative CTA and PCS (P= .017 and <.001, respectively).
Conclusions:
The Iris registration can minimize astigmatic under-correction in topography-guided laser vision correction. Greater benefit is expected in cases with higher topographic astigmatism and larger horizontal angle kappa.
