Purpose:
To determine that multifocal implants can be offered to a select group of well controlled glaucoma patients whose intraucular pressure(IOP) is controlled after selective laser trabeculoplasty(SLT) or by a single glaucoma medication .
Setting:
A clinical private practice setting
Methods:
14 eyes with were well controlled IOP after SLT or a single medication underwent cataract surgery with multifocal IOL implantation after detailed discussion of risks and benefits. All eyes had well controlled target IOPpre-operatively..Astigmatism greater than 0.5diopters was corrected by limball relaxing incisions(LRI). All patients on glaucoma medications continued them post operatively in addition to standard post -op medications. Patients taking prostaglandin analogues pre-operatively were switched to other medications pre operatively or underwent SLT to reduce IOP to target IOP.. All received a minimum of 4-7 days of Bromfenac prior to and continued for 4 weeks after surgery.
Results:
All eyes achieved uncorrected 20/40 and J3 or better uncorrected vision with IOP remaining controlled with the same medications post op. 3 eyes received additional SLT. 2 needed additional medications for short period for temporary control of elevated IOP.3 eyes had previous filtering surgery and the IOP remained controlled with functioning blebs.7 eyes had reduction of IOP from pre-op level and were taken off medication.No eyes had intra operative complications. All eyes had improvement in vision and no eyes sutained visual field loss.
Conclusions:
Multifocal IOL's can be implantedin a select group of well controlled glaucoma patients.. FINANCIAL DISCLOSURE?: No
