Sodium hyaluronate and trabeculectomy

Purpose:

To attempt to prove the efficacy of subconjuctival sodium hyaluronate in trabeculectomy

Setting:

Arjeta Grezda, Ermal Simaku, Luljeta Jaupaj Eye Clinic, University Eye Hospital " Mother Theresa" Tirana, Albania

Methods:

20 eyes from 20 patients affected by POAG underwent glaucoma surgery-routine trabeculectomy, with 10 patients of study group receiving sodium hyaluronate injected between the sclera and the conjuctival flap at the conclusion of the surgery; and 10 patients of the control group receiving a similar injection with balanced salt solution. Intraocular pressure and bleb morphology were evaluated in the two groups. Success rate was considered IOP<21 mmHg without any glaucoma medication and optimal bleb morphology was considered the bleb type I, the multicystic bleb.

Results:

Complete success rates were 80.2% for the study group and 85.8% for the control group 6 months after the surgery. Regarding the bleb type 92% of the study group has diffuse-multicystic bleb type while 86.7% of the control group has diffuse-multicystic blebs. The blebs in the study group were studied and classified in two groups: in the study group 56.1% of the eyes had cystic blebs and 43.9% had diffuse blebs, while in the control group 38.8% had cystic blebs and 61.2% had diffused blebs.

Conclusions:

The study found that subconjunctival sodium hyaluronate does not increase the intermediate-term success rate of glaucoma filtering surgery. However, this approach is associated with the formation of more cystic- diffuse blebs. The small sample size should be considered before making generalization on these findings.
