Purpose:
To present 4 cases with endothelial cell loss long time after Artisan pIOL implantation for the correction of myopia.

Results:
We present 4 cases with severe endothelial cell loss after implantation of an Artisan phakic intraocular lens for the correction of myopia. The mean age of the cases is 60 years with a preoperative spherical equivalent of S-14.97±5.18 (mean±standard deviation) diopters and an anterior chamber depth 3.38±0.35 mm from the corneal epithelium.
After a follow up of 10.38±3.86 years, we observed an endothelial cell loss per year of 10.76±15.35% in these 4 cases. With Pentacam we measured a nasal, temporal and central critical distance of respectively 1.24±0.41, 1.28±0.28 and 1.7±0.33 mm. Risk factors as rubbing eyes and contact sports were evaluated. One patient answered positive for chronic eye rubbing.
Conclusions:
The minimal critical distance should be carefully re-evaluated to establish a sufficient safe zone of minimum clearance between intraocular lens and corneal endothelium. Patients should be thoroughly instructed to follow a regimen where risk factors for endothelial cell loss can be minimized.
