Postoperative hypertensive phase occurrence and management following Ex-PRESS™ glaucoma drainage device implantation

Abstract Details

Purpose:

To report on the postoperative hypertensive phase (POHP) and its management, following Ex-PRESSTM Implantation.

Setting:

Ein Tal Eye Hospital, Ein Tal Eye Hospital, Tel Aviv Israel.

Methods:

Patients that needed filtration surgery alone or combined with cataract extraction were prospectively enrolled for Ex-PRESSTM implantation under a scleral flap using Mitomycin C. The postoperative course was reviewed and analyzed to determine the occurrence of a POHP and its eventual management. Outcome measures included intra-ocular pressure (IOP), number of anti-glaucoma medications, and complications.

Results:

From February 2008 till December 2011, 158 eyes that needed filtration surgery were implanted with the Ex-PRESSTM. 107 eyes (68%) experienced a transient POHP (IOP>18mmHg without antiglaucoma medications) that occurred from the second week and the sixth week postoperatively. In 62 eyes (39%) the IOP was >18<25mmHg and returned to <18mmHg spontaneously within four weeks of the POHP onset. In 45 eyes (28%) the IOP was >25<40mmHg and necessitated transient antiglaucoma medical treatment and/or minor surgical interventions to lower the IOP.

Conclusions:

The POHP is a common and generally transient occurrence, between the second and sixth week after Ex-PRESSTM implantation. Medical treatment or minor surgical interventions become necessary only when the IOP is >25mmHg.
