Six-month experience with excimer laser treatments for simultaneous alleviation of presbyopic symptoms together with correcting the refractive deficit

Purpose:

To investigate the outcomes for simultaneous alleviation of presbyopic symptoms together with correcting the refractive deficit by means of bilateral LASIK treatments.

Setting:

Dr. M.C. Arbelaez, Muscat Eye Laser Center, Muscat, Sultanate of Oman.

Methods:

All patients underwent presbyopia treatment with the Schwind AMARIS excimer laser (SCHWIND eye-tech-solutions) using the micro-monovision approach of the PresbyMAX software. LASIK flaps were cut using LDV femtosecond (Ziemer) laser in all cases. All treatments had been non-customized using the corneal vertex for ablation centration. Clinical outcomes were evaluated in terms of predictability, refractive outcome, safety, corrected and uncorrected visual acuity for near, intermediate and far vision. Separate analysis for myopic, hyperopic and emmetropic groups had been performed. Enhancement rate was less than 10%.

Results:

Pre-operative refraction was in the range of +4.0 D to -8.0 D and astigmatism was smaller than 4.0 D. Near addition for all patients had been in the range from +1.0 D to +2.5 D. Post-operatively the uncorrected visual acuity improved significantly at all distances. Distance vision and patient overall satisfaction increase with time and distance eyes end up close to emmetropia. Near eyes end up between -0.5 D and -0.75 D. The worst distance eye could see J6 UNVA monocularly and the worst near eye could see 20/40 UDVA monocularly. Astigmatism correction was as good as in conventional AMARIS treatments. No differences among myopes, hyperopes, emmetropes or between males and females were found.

Conclusions:

LASIK with the micro-monovision approach of the PresbyMAX yield superior outcomes. Presbyopic symptoms were post-operatively significantly reduced by means of pseudo-accommodation. Within the first three months the changes in refraction and visual acuity can cause some discomfort. Results were stable from the third month follow-up, providing excellent distance vision, very good near vision and good vision at the intermediate region.
