Purpose:
To compare and evaluate the predictability, stability, safety, efficacy and change in ocular aberrations after wavefront optimized LASIK with ?ap created by femtosecond laser (iFS 150 Hz) and mechanical microkeratome One Use Plus SBK (Moria SA).

Methods:
Design : Prospective , nonrandomized, comparative clinical study
100 eyes (50 patients) with myopia and myopic astigmatism were distributed into 2 groups i.e. 50 [25 patients] eyes for each group. Patients in group A had LASIK flap creation with iFS (Intralase femtosecond laser 150Hz) and in group B with One Use-Plus SBK (Moria SA). All eyes underwent wavefront optimized LASIK on MEL 80 [Zeiss] excimer Laser. Preoperative , one month and 3 month postoperative wavefront errors from the whole eye (WASCA analyzer, Carl Zeiss, Germany) were calculated over 4mm and 6mm diameter and decomposed into Zernike polynomials to the 6th order.
Results:
Both groups were matched in terms of age, sex, spherical equivalent, pachymetry and keratometry total higher order, spherical and coma aberrations. Both procedures were safe, efficacious, stable and predictable to the same extent. Visual recovery was quicker in femtosecond group. In first post operative week 88% eyes in femtosecond group achieved postoperative UCVA of 20/20 or better which was significantly higher [<0.001] than in the mechanical micro- keratome group[54%] . At the end of 3 months 48% achieved the UCVA of 20/16 in femtosecond laser group against 33% in microkeratome group. Post operative total high-order aberrations, spherical and coma aberrations increased significantly from pre-operative values [p< 0.000] in both groups, but there was no significant difference between two groups at the end of one month and 3 months post-operatively.
Conclusions:
Visual outcomes of wave-front optimized LASIK procedure with MEL 80 , in which ?aps were created with either femtosecond laser (iFS 150Hz, AMO) or mechanical microkeratome (Moria Evo3 One Use-Plus, Moria SA) were same in terms of safety, efficacy, predictability, stability and induction of higher order aberrations at the end of 3 month follow up.
