Small incision cataract surgery versus phacoemulsification for hard cataract

Purpose:

To compare efficacy and safety of manual small incision cataract surgery (SICS) versus Phacoemulsification to treat mature and hard leathery cataract.

Setting:

Bawri Nethralaya, India

Methods:

Duration of study was one year. Group A included 40 cases of SICS and group B included 40 cases of Phacoemulsification performed with direct chop technique. Group B was subdivided in to two groups of 20 cases each using vertical and horizontal chopping. Surgical complications, operation time, post operative vision and quality of vision were compared.

Results:

Transient corneal edema was more common with group B irrespective of phacoemulsification technique (p<0.05). Operation time was significantly shorter with group A. Visual acuity was comparable in both the group at the end of 1 week and 4 weeks. The quality of vision was better in group B (p<0.05) because of less surgically induced astigmatism and better quality of intraocular lens. Posterior capsular rent was 2.5% with SICS and 5% with vertical chopping.

Conclusions:

Phacoemulsification has upper edge over SICS for mature cataract in terms of quality of surgery and vision. However, one should perform phacoemulsification for hard lathery cataract very cautiously. FINANCIAL DISCLOSURE?: No
