Purpose:
to compare the surgical results of either Phacoemulsification alone or combined phacotrabeculectomy with intraoperative application of mitomycin c in the surgical treatment of medically controlled chronic angle closure glaucoma (CACG) with coexisting cataract.
Setting:
Benha University Hospital, Benha, Egypt
Methods:
42 eyes of 42 patients from the outpatient clinic of Benha university hospital were included in this randomized clinical study .They were equally divided into group 1 (Phacoemulsification alone) and group 2 (combined phacotrabeculectomy with adjunctive mitomycin C (MMC)). The patients were evaluated one month and every 3 months for two years, regarding intra ocular pressure (IOP) measurement, necessity for anti glaucoma drugs, and any complications or glaucoma disease progression.
Results:
The mean IOP showed no statistically significant difference (p <0.05) between both groups preoperatively and postoperatively except at 1m (p=0.002) and 3m (p=0.007).Group 2 showed statistically significant less number of medications at the end of the study by 0.7 (p<0.001). Group 2 was associated with more postoperative complications than in group 1.
Conclusions:
combined phacotrabeculectomy with adjunctive mitomycin C can achieve better IOP control than Phacoemulsification alone in medically controlled CACG eye with cataract. However it may be associated with more complications and additional surgical interference. FINANCIAL DISCLOSURE?: No
