Combination of quadrangular (of BE Malyugin) and hooked polymeric iris-retractors in surgery of the cataract complicated by weakness of irido-crystalline lens diaphragm and inosculated pupil

Purpose:

The research aim is to estimate efficiency of the combination ¬ of quadrangular and hooked iris-retractors at carrying out of phacoemulsification on eyes with the expressed weakness of irido-crystalline lens diaphragm and inosculated pupil.

Setting:

During the period since 2010 8 eyes of 8 patients (6 women and 2 men) were operated in the State Institution " Republican Scientific and Practical Centre of Radiation Medicine and Human Ecology" with the diagnosis: the secondary post-uveal mature cataract complicated by the expressed weakness of irido-crystalline lens diaphragm and inosculated pupil. The average age of patients is 42,3±5,9 years.

Methods:

The patients were performed the following researches: visometry, tonometry, keratometry, pachymetry, refractometry, US-biometry. The optical coherent tomography of the front piece was made, the calculation of optical force of the intra-ocular lenses was made on the device " IOL-Master" . All operations were carried out on phacoemulsificator " InfinityVisionSystemOzil" (?lcon, the USA). Before division of the front circular synechia the surgery was carried out by a standard technique. Then the implantation was made ¬of the quadrangular iris-reactor of B.E.Malyugin ¬for pupil mydriasis. The visualization of the front capsule was performed by blue solution 0,06 % (India). At presence of fibrosis of the front capsule the vitreal scissors 20G were used. The capsule bag was weighted in all cases on hooked iris-retractors. The nucleus break was made by technique phaco-shop. After implantation of IOL and hydration of cuts the solution " Ophtacvix" was injected.

Results:

A clinical case: the patient of 35 years old. The diagnosis of the right eye: ¬ mature complicated post-uveal cataract, inosculation of a pupil, fibrosis of the front capsule of the crystalline lens against the secondary operated glaucoma. The initial visual acuity is pr.luc.certa. The adventitious device of an eye is without features. The cornea is transparent. The front chamber is smaller then the average depth, uneven, the iris is atrophic, at 11.30 the basal colomba, cat-eye pupil 1x2 mm, the circular synechia. The reaction of a pupil to light is not defined. The crystalline lens was lacklustre. Iridophakodonesis is defined. V-scanning: OU-initial vitreal destruction. The tunics are normal and adjoined. Gonioscopy: a zone of the sinus trabeculectomy is sclerosed, the angle is uneven, a radical atrophy of iris stroma. The postoperative visual acuity is 0,2 and stable.

Conclusions:

Using of the combination of iris-retractors allows to achieve high anatomic and functional ¬results in surgery of post-uveal cataract with the expressed weakness of iridocapsular diaphragm and inosculated pupil even in serious cases. FINANCIAL DISCLOSURE?: No
